Remote Patient Monitoring (RPM) is an additional service line Pinnacle Family Medicine is now offering and pairs well with the CCM program. RPM is also a program covered by insurance.
We offer blood pressure monitors, a digital scale, glucometer and/or pulse oximeter. If you have a diagnosis or health condition that qualifies you for one of these devices, we can order the device and have it shipped directly to your home.
Once you begin to take readings, they automatically transmit to a care manager, and we monitor them and intervene if needed. We can generate a chart of these readings for Dr. Crowell to use during her visit with you and even provide this record for you to take to appointments with specialists.
